Precast concrete walls

Anyone have any idea what causes these rust stains on this commercial building? I'm pretty sure how to remove it but the people that own it are convinced it is from nail heads. It appears to have a cavity like a rock with iron decomposed and left a void.

They use redhead bolts, epoxied into the wall, to hold the temporary braces to support the wall. Might be the bolt is / was still in the wall and rusted.

Or, could be a nail left over from the form, if the walls are not pre fab.

I think we turned this building down this morning. I may have some info for you on the rust. Give me call tomorrow if you're interested.
